Breast Augmentation
The fascination of both sexes with the female breast and the desire by women to have fuller breasts predates the writing of the Bible. Dating back to antiquity, the written records, paintings and sculptures of many cultures have idealized the shapely breast.

Youthful visible breasts have always defined feminine attraction but have increasingly done so with the advent of Hollywood and movies. Many of our favorite female movie stars, both past and present, amply demonstrate this feature. With the massive popularity of films just before- and especially, after WWII, Hollywood and glamour magazines have intensified the pressure on women with small breasts by repeatedly showing what it considered was the ideal breast shape. However, until the mid 20th century, Plastic Surgery did not have the capability to predictably attempt correction of many of nature’s shortcomings. Breast enlargement is a great operation which may readily improve the appearance and reverse the insecurity and self-doubt commonly accompanying women with smaller breasts. In the past 30 years, nearly two million American women had undergone breast augmentation and the vast majority are happy with their decision.
Small or mildly sagging breasts can be made perkier and larger by placement of implants filled either with saline (sterile salt water) or, in some patients, silicone gel. The implants are commonly placed through a small incision either under the breast fold (inframammary), around the nipple complex (periareolar) or through the armpit (trans-axillary).

| Breast Implant Placement |
|
The implants may be placed either under the breast gland and over the muscle, or under the pectoralis muscle. The Plastic surgeon then creates a pocket, either directly behind the breast tissue or underneath your chest wall muscle. Implant placement behind the muscle (retro-pectoral) may interfere less with mammograms than if the implant is placed directly behind the breast tissue. The choice of incision and site of implant placement depends on the patient’s goals and expectations.
There are many options in breast augmentation without a single "best choice" which is universally applicable to all women. Each choice has its own advantages and trade-offs. Since each woman possess a unique combination of anatomy and long-term goals, we strongly believe that breast augmentation must be individualized and customized to meet each individual woman's circumstances rather than using a "one operation fits all" approach. Your best choice will be determined by fully understanding all these choices.
There are many different types of breast implants. In general, breast implants are classified based on their shape (round vs. teardrop (i.e. anatomical)), their filler (saline vs. silicone) and the texture of their surface (smooth vs. textured).
Textured breast implants were engineered to prevent their moving (i.e. displacement) or turning (i.e. rotation) in the breast pocket but have higher risk of breast rippling. Unfortunately, textured implants cost more than smooth implants and are also associated with a higher deflation rate and are firmer than smooth implants. Smooth breast implants have a lower risk of rippling, are less firm than textured breast implants and any rotation of the implant will not affect the appearance of the breast.
Round implants are the most commonly used breast implants. They usually have a smooth surface (which causes less wrinkling), are not affected by rotation and cost less than teardrop implants. Teardrop implants were developed to provide a more natural look. They cost more than round breast implants and require a textured surface to prevent their rotation or displacement. In the event that the implants rotate, it the breast shape will appear unnatural.
A recent study determined that a round breast implant takes on the same shape as the teardrop implant when standing. The study also concluded that when lying down, the round breast implant appears more natural in appearance than the teardrop implant because it retains the teardrop shape and the round breast implant does not.
The procedure usually lasts 1 to 2 hours and is done as an outpatient procedure either under local anesthesia with sedation, or under general anesthesia. You may have temporary soreness, swelling, bruising, change in nipple sensation. You're likely to feel tired and sore for a few days following your surgery, but you'll be up and around in 24 to 48 hours. Most of your discomfort can be controlled by medication prescribed by your doctor. The breast may be sensitive to stimulation for a few weeks. Most women return back to work in a few days and to more strenuous activities in 3 to 4 weeks.
Potential complications are bleeding, infection, formation of noticeable scar tissue around the implants (capsule contracture) which may cause the breast to feel tight, undesired implant shifting or movement laterally, waviness or slight rippling of the upper breast skin or changes in nipple sensation which may be permanent. The most common complaint with breast augmentation is that the end-result is not as large as envisaged. However, big implants impose their own problems. The larger the implant, the heavier it is - resulting in faster sagging of the breast. In addition, with saline-filled implants, which are particularly prone to wrinkling, more rippling is seen at the surface of the skin where muscular coverage is either thin or missing. While the majority of women do not experience these complications, you should discuss each of them with your surgeon to make sure you understand the risks of breast augmentation.
